About Us
Community Options provides a Palliative Care service that delivers intensive case management and in-home and community-based support services (non-clinical) for people diagnosed with end-stage illnesses who wish to die at home.
We actively assist people to access appropraite palliative care services in order to assist with improving quality of life outcomes for people with end-stage illnesses and their families, by relieving pressure and reducing the stress associated with caring for a family member with an end-stage illness.
This service is accessible to:
- ACT residents with diagnoses of life-threatening end-stage illnesses;
- All age groups;
- People recieving home based palliative care services (clinical services);
- People with complex needs requiring a complex service/case management service response that cannot be met within the existing services.

Community Options is an ACT NDIS registered provider. We are a not-for-profit community based organisation with more than 25 years of experience in providing services to older people, people with disabilities and their families and carers. We provide Level 1, Level 2 and Level 3 of supports connection and supports coordination and plan management for people of all ages with complex needs, psycho-social disability, and/or degenerative diseases (such as Younger Onset Dementia, MS, MND). We provide a wide range of short term and long term in-home and community based support services. Services include personal care, social support, community access, domestic assistance, case management and coordination.
