Footprints In Brisbane Inc - Fortitude Valley

Footprints provides case management and in home support for frail aged and younger disabled people and their carers. They also provide Housing Linked Community Care packages, Community Options program, basic care packages, centre based respite and mental health services. Footprints also offers a homeless outreach program for frail aged and younger people with a disability who are homeless and a Residents Support Program for residents living in supported accommodation and boarding houses.

Footprints is a not-for-profit provider of community based services in Brisbane recognised for quality service and individualised care. We offer an integrated range of programs and services, for people residing in the community throughout North Brisbane
as well as home-based services for older persons within inner Northern and Southern Brisbane.

Eligibility
• Medical referral not necessary for the Homeless Outreach Program or Drop-in
• Referrals also accepted from family, friends and community services and health professionals
• Meet criteria for Commonwealth Home and Community Care Program for people over 65 years and the Qld
Community Care Services for those under 65.

Services include assistance in obtaining and sustaining housing, street outreach, individual case management, counselling and support, advocacy and mediation, information and referrals, ongoing individual support. A Drop-in friendly and safe place is also operated every Wednesday from 7.30am to 1pm where lunch is provided by gold coin donation, there is use of showers, laundry facilities, a pool table and computers, and tea, coffee and a chat. Access is also provided to the Centrelink Community and Homeless Health Outreach Teams and the Anglicare Nursing Service.
