Chermside Community Health Centre provides the following services: * Mental health; * Adult community health; * Diabetes; * Indigenous Health; * Alcohol and Drug Services; * Aged Care Assessment Service; and * Acute Care Team. Chermside Community Health Centre offers access to Home Care (HACC) programs, a joint state and federal government initiative which provides frail aged people and people with a disability access to practical services which assist them to continue living in their own home. Support services are also available for carers. Services which may be available to eligible clients include domestic assistance, social support, home nursing and personal care, respite care (in home and centre-based), home maintenance or minor modifications, allied health services, dietitian, speech pathology, physiotherapy and referral to other support services where needed (such as Meals on Wheels). Fees apply to certain HACC services, but special arrangements may be made for people who cannot afford these costs. Alcohol and Drug Service provides assessment and counselling for individuals and significant others. Diabetes Service provides after assessment education groups, dietician, and podiatry and nurse educators.
