About Us
Description
· TAKE HOLD is a Mentoring and Living Skills Program. This service is part of a suite of programs working collaboratively, aimed at substantially reducing the ACT homeless population. The primary aim of the program is to support young people (16-25) who are transitioning into or out of homelessness, to develop the life skills necessary to connect them into their community and avoid homelessness. This is done through mentoring, living skills development and case management. We run living skills groups and work one to one with clients. We also can provide mentors to work with young people one to one.
- The mentor works collaboratively to support an individual mentee in the development of their skills and capacities as they redevelop their healthy connection with community.
Purpose
- To support young people experiencing homelessness.
- To help the young person to engage or reengage with education/training
- To help the young person to engage or reengage with employment
- To help the young person to reengage with their family
- To help the young person connect or reconnect with their community
- To support the young person as they progress through crises accommodation to transitional accommodation into a Housing ACT tenancy, community housing or private rental tenancy
- To help the young person obtain, maintain and sustain their respective tenancy
Target Group
- Young People aged 16-25 transitioning into or out of homelessness
Program Information
- Mentoring: 1 to 2 years, 6-10 hours a month
- Groups: 1-2 hours
- Living Skills One to One: time is determined by needs
Program Capacity
- For all programs referrals are a must
- No limit on service delivery
- Many group topics can be covered including arts and music based topics
