About Us

Description

· TAKE HOLD is a Mentoring and Living Skills Program. This service is part of a suite of programs working collaboratively, aimed at substantially reducing the ACT homeless population. The primary aim of the program is to support young people (16-25) who are transitioning into or out of homelessness, to develop the life skills necessary to connect them into their community and avoid homelessness. This is done through mentoring, living skills development and case management. We run living skills groups and work one to one with clients. We also can provide mentors to work with young people one to one.

  • The mentor works collaboratively to support an individual mentee in the development of their skills and capacities as they redevelop their healthy connection with community.
