Centacare - SWBCOP - West Moreton

Marginalised Community Aged Care Packages program. This program provides accessible and affordable services for individuals residing in boarding houses/caravan parks and other accommodation, who may be at risk of homelessness. The packages are targeted to frail aged people aged 70 years and older and 50 years and older for Aboriginal Torres Strait Islander people. In special circumstances persons of other age groups who are financially and socially disadvantaged may be considered eligible. To access a package all persons must be assessed as eligible by an Aged Care Assessment Team (ACAT). Range of services available include: • personal care • medication support • domestic assistance • support to access the community (e.g. shopping) • social inclusion (e.g. group outings) • referrals • information Valuing Respect, Culture, Choice, Dignity, Diversity and Independence
